What We Need
To schedule your patient's imaging, please include the following with your order:
- Patient demographics (name, DOB, contact)
- Exam requested (X-Ray, Ultrasound, EKG)
- Dx / ICD-10 code (if required)
- Ordering provider name + signature
- Site contact + best time window for visit
